Record Request (Online)

If you need copies of a property's current septic and well records, please complete the form below. For record look-ups, the following information is required: 

• provide the current property owner name and complete address
• if possible, provide the original septic/well permit holder's full name
• provide any associated documents if applicable  

 *Record look-ups cannot be completed without this correct information.

 **A $25.00 fee is required prior to the request being completed. Please call (304)788-1321 for billing information.

Please allow 3-5 business days for record look-up. Records will be emailed to the address provided in the form. You may also request records by fax 304-788-6023.
